Implants or Your Own Fat
The first decision you make is what adds the volume, an implant or your own fat, and the two routes compare like this.

Fat transfer generally suits you if you want a modest change, have some spare fat to move and keep a stable weight. Implants suit a larger or more defined change. Your surgeon recommends the route that fits your goals and your body at your consultation.
Choosing Your Implants
Four choices shape your result: size, profile and shape, placement and the incision route. Your surgeon talks you through each one.
Size: Start From the Look You Want
Cup sizes vary between bra makers, which makes them a loose target. The same implant volume also looks different on a narrow frame than on a broad one. A number on its own tells your surgeon little about what you want.
Bring photos of results you like, so your surgeon can see the look you are after. Your surgeon then measures your chest and helps you choose a volume that fits your frame and that look.
Profile and Shape
Profile describes how far the implant projects forward from your chest. At the same volume, a higher profile is narrower and sits further forward. Manufacturers name their profiles differently, so if you compare quotes, compare the look rather than the label.
Shape is the other half of the decision. Round implants add fullness across the breast, while teardrop (anatomical) implants slope more gently and usually have a textured surface. Implants also come filled with silicone gel or saline, and your surgeon explains which suits you.
Over or Under the Muscle
An implant can sit under the chest muscle or over it, and each position has its case.
Under the muscle, the implant has more cover, which helps hide its edges when your own breast tissue is thin. Scar tissue tightening may also be less likely, though the first few days tend to be more uncomfortable. In practice the implant usually sits partly under the muscle (the top beneath it, the lower part under your breast tissue), which is called dual plane.
Over the muscle, the implant sits where your breast tissue already lies and moves less when the chest muscle works. That is why it often suits very active people and people who lift weights. It needs enough of your own tissue to cover it, because its edges can show more and scar tissue tightening may be more likely.
Which one suits you depends on your tissue, your goals and how you live, and your surgeon explains the reasoning.
Where the Incision Goes
There are three routes in, and surgeons differ on which they prefer.
- The crease under your breast (the inframammary fold): A common route, with the scar sitting in the crease where the underside of the breast meets your chest.
- The edge of the areola (periareolar): The scar sits where the darker skin meets the lighter skin, and your surgeon considers the size of your areola when recommending it.
- The armpit (transaxillary): The scar sits in the armpit rather than on the breast, but the route limits the implant type and size your surgeon can place.

Who Breast Augmentation Suits
Breast augmentation suits you if your breasts feel small for your frame, differ in size or have lost volume over the years.
If your breasts have dropped as well as lost volume, a breast lift may suit you instead of implants, or alongside them. Your surgeon examines you and says which.
A few criteria apply:
- You are in good general health.
- Your breasts have finished developing.
- You are not pregnant or breastfeeding at the time of surgery.
- You are ready to stop smoking around the operation, and your surgeon tells you for how long.
If breast cancer runs in your family, tell your surgeon, who may ask you to have a breast scan first.
Meet Our Breast Augmentation Surgeons
Two of our surgeons perform breast augmentation, and either of them may be your surgeon.
Dr. Wutthiwat “Tei” Anupansawang, MD (Medical Council of Thailand ว.30963) has 22 years of experience and holds a board certification in general surgery. He works in body contouring surgery, and the procedures he performs include breast augmentation, liposuction and abdominoplasty.
Dr. Pawalin “Palm” Chuenputhi, MD (Medical Council of Thailand ว.31884) has 21 years of experience and is board certified in both general surgery and plastic surgery. Her clinical experience covers facial and breast surgery.
At your consultation your surgeon examines your chest and breast tissue, then talks you through size, placement and incision before explaining the plan.
What Happens on the Day
All surgery at KKC Clinic takes place at our surgical facility in Ekkamai. On the day, your surgeon goes over the plan with you and marks your chest, then you have general anesthesia, so you are asleep throughout.
With implants, your surgeon makes the incision agreed at your consultation and creates a pocket over or under the muscle. Your surgeon then places the implant and closes the incision with stitches.
With fat transfer, your surgeon first removes fat by liposuction from the agreed area and prepares it. Your surgeon then injects it into your breasts in small amounts.
You wake with dressings and a support bra or garment in place, and our team monitors you as the anesthetic wears off.
Recovery
Your surgeon gives you your own recovery plan, and most people follow a similar pattern.
- Support bra: Wear the support bra or garment day and night for as long as your surgeon advises, often several weeks and sometimes up to about three months.
- Discomfort: It is usually strongest in the first few days and eases with the pain relief your surgeon prescribes.
- Swelling and settling: Swelling settles over the first weeks. Implants often sit high and feel firm at first, then soften and settle into place over a few months.
- Wounds and showering: Keep your wounds dry and covered as your surgeon instructs, and shower when your surgeon says you can.
- Sleeping: Sleep on your back with your head raised in the early weeks, not on your front.
- Exercise: Gentle walking helps from early on. Leave heavy lifting and strenuous exercise for around six weeks, or as long as your surgeon advises. Bring chest exercise back last, especially with implants under the muscle.
- Check-ups and questions: Your surgeon checks your wounds before you fly home and sets the date. If you have a question at any point, message our consultant team through the usual messaging channels.
Coming to Bangkok for Surgery
Plan on staying in Bangkok for five to seven days after the operation, or longer if you combine it with another procedure. Before you fly, your surgeon checks how your wounds are healing and confirms when you are fit to fly.
Pack clothes that fasten at the front, because raising your arms is uncomfortable at first, and plan a quiet first few days. Avoid swimming and long spells in the sun until your surgeon says your wounds have healed.
For the flight, book an aisle seat, drink plenty of water, move about the cabin and ask your surgeon whether to wear compression stockings. If you take hormonal contraception or have ever had a blood clot, tell your surgeon at your consultation.

Risks
Every operation carries risk, and these are the ones for you to understand before breast augmentation.
- Capsular contracture: Scar tissue forms around every implant, and in some people it tightens, changing the shape of the breast or causing pain. In long-term studies, roughly 1 in 10 to 1 in 5 people had this within 10 years, and it may need further surgery.
- Rupture and rippling: An implant can leak or break, and a silicone leak may cause no symptoms, which is why regular scans are recommended (see the next section). Rippling means the implant’s edges show or can be felt, more often when the tissue over it is thin.
- Shape and sensation: The implant can settle too low or your breasts may end up slightly uneven, which sometimes needs further surgery. Nipple and breast sensation can also change, and while it often improves over months, it can be lasting.
- Infection, bleeding and clots: Any operation under general anesthesia carries some risk of infection, bleeding, fluid build-up, blood clots and anesthesia problems. An infected implant sometimes has to be removed.
- Rare conditions: BIA-ALCL is an uncommon cancer of the immune system, and almost all cases have involved textured implants. BIA-SCC is a very rare cancer of the scar capsule, reported with all implant types. Breast implant illness describes symptoms some people report, such as tiredness and joint pain, whose cause has not been established.

How Long Implants Last
Breast implants are not lifetime devices, so it helps to know what that means for you before you choose them. There is no fixed date to replace them.
Many people keep theirs for many years. Others have further surgery because of a complication, a change in their body or a wish to change size. In one long-term study, about 1 in 4 people had another operation within 10 years.
Advice on checking silicone implants varies by country. In the US, the FDA recommends an ultrasound or MRI scan 5 to 6 years after surgery, then every 2 to 3 years, even if you have no symptoms, because a silicone leak may not cause any. Saline implants do not need these scans, because a leak usually shows as a change in size.
Keep a record of your implant’s brand, model, size and surface, since future scans and any recall will need it. Revision surgery is available when you want or need it, and your surgeon can discuss what it involves.
